After placing your order, you will receive an email with a short survey to collect the information for the Certificate of Authenticity, presented as a toe tag and included with your mug.
The following details, provided by the collector, will be recorded on the toe tag COA:
Name of Deceased, Age, Sex, Race, Weight, Height, Place of Death, Date of Death, and Cause of Death.
